DBPR: Adding/Removing Your PA, LLC, PL or PLLC Designation

Here are the instructions on how to add or remove your designation with the DBPR.

If you do not have a copy of your PA, LLC, etc. you will need to download your paperwork from the Sunbiz website as follows:

      -Log in to Division of Corporations Sunbiz.org website at: www.sunbiz.org

                     -Click on “Search Records” and type in your PA, LLC, etc.

      -Download/save the records to your PC.

      -Log in to the DBPR website at:  www.myfloridalicense.com

- Once logged in click on “Apply for a New License”.


- Click on “Real Estate Commission”.


-Click on “Apply for A License”.


-Click on “Manage My License”.


-Click on “Add/Remove PA, LLC, PL or PLLC - Sales Associate or Broker Sales Associate (Form RE 16)”.

-Click "Apply Using a Printable Application" - bottom of screen to the left:

-Save the form to your desktop & print out a copy.

-Fill out the application, attach your PA, LLC, PLLC, etc. paperwork (document showing your PA, LLC, PLLC, etc. as well as the Articles) from SunBiz.org.

-Send signed application & forms along with your check for $30.00 and mail to the address on Pg. 1 of the application.

-Ensure you make a copy of the application prior to mailing it!

-Once your application & check are received by the DBPR & processed, your designation will be added to your name.

-Your designation will now appear in the DBPR next to your name (example:  John Smith, PA or John Smith LLC).

-Once approved by the DBPR, please print out a new blank W-9 with your name and designation & tax-ID or EIN#.

-Here is the link: https://www.irs.gov/pub/irs-pdf/fw9.pdf  to a blank W-9 (see below) that you can fill out online, save to your PC and send a signed copy to Miles.



*You must have filed your PA or LLC with Sunbiz prior to filling out the application with the DBPR. If you have not, you will need to do so with Sunbiz by filling out the application and paying the fee. Sunbiz can take up to 4 weeks to record the PA or LLC so plan accordingly.
